The Sphera platform: a shared technological foundation for precision medicine

Sphera is the shared architecture on which we build our solutions: ingestion of clinical and molecular data, a knowledge and rules engine, a traceability layer and report generation. On that foundation we develop vertical solutions for specific clinical domains, each with its own care logic and workflow. Sphera OncoCore, designed for tumor boards, is the first.

How it is organised

The platform is structured in five layers, from input data to hospital integration. Each layer is independent: it can be improved, audited or adapted without altering the rest.

    Data layer

    Ingestion and normalisation of clinical and molecular data: sequencing reports, clinical variables and case documentation.

    Knowledge and rules engine

    Clinical knowledge bases, reference frameworks (AMP/ASCO/CAP, NCCN, ESMO) and pharmacological rules applied consistently to every case.

    Traceability and audit layer

    Every statement generated is linked to its original source, so the system's reasoning can be followed and verified step by step.

    Vertical solutions

    Products for specific clinical domains built on the shared foundation. OncoCore, for precision oncology, is the first.

    Input and output formats and exchange logic that adapt to the information systems of each centre.

The first vertical solution: Sphera OncoCore

OncoCore applies the Sphera foundation to the domain where information overload is most evident: precision oncology. It processes next-generation sequencing (NGS) panel results, classifies the actionability of findings according to AMP/ASCO/CAP, checks the pharmacological safety of each option against CIMA, EMA, FDA and BIFIMED, makes explicit the clinical variables still to be confirmed, and delivers a traceable report to the tumor board with every statement linked to its source.

Its current clinical scope is non-small cell lung cancer (NSCLC), and the system is under active development, open to clinical evaluation with collaborating services.

Under development

The Sphera architecture is ready to extend to other clinical domains by reusing the shared layers and building the logic specific to each specialty on top of them. Our ongoing work focuses on two fronts:

Extending OncoCore to other tumour subtypes

With the knowledge base consolidated for NSCLC, the roadmap foresees progressive expansion, prioritising tumours where molecular interpretation weighs most heavily on the board's decision. The order of that expansion is defined together with the clinical teams we work with.

New modules on the shared foundation

With our clinical collaborators we assess which other domains could benefit from the same combination of data ingestion, knowledge engine and traceability. We do not announce modules until their scope is defined and their evaluation under way: we prefer to commit only to what we can sustain.

Why modularity matters in a hospital

Because no two hospitals work the same way, and we develop the platform ourselves. That makes Sphera malleable: the interface, the input and output formats and the integration logic adapt to the protocols, workflows and information systems of each service, instead of forcing the service to adapt to a closed product.

The same modularity allows capabilities to be added in phases — starting with molecular interpretation and adding pharmacological checks or integration with the electronic health record viewer later — and keeps each piece independently auditable.

In summary

Sphera OncoCore is a platform under development, intended for research and clinical evaluation purposes. It does not hold CE marking as a medical device and is not intended for use as a diagnostic or prescribing tool. Every clinical decision remains the responsibility of the treating healthcare professional.
